[issue678] patch cant apply changes at head of file (without prior context)

hallow developers!
i got an conflict when try to apply the patch on new version of code. 
the change was in ChangeLog file and contain some insetrion into head of log.
therefore in the patchfile - delta have no prior context, and only have an 
context after insertion.
the patch would can found the place where to insert chages by post-context, 
but it rejects this changes instead.

IMHO mercurial patch would able to resolve insertion to head or 
appending to tail - changes, or at least provide more conrol to user on it.
