[issue649] Strange I/O error on committing on win32

Due to cset:8a53b39cd402 (r4902 in crew),
strange I/O error occur on committing many files.
I've experienced on simply importing expanded tarball sources.

I bisect'ed and found that this error is caused by cs:8a53b39cd402 
and I guess '+' in mode should allow GENERIC_READ right (in util_win32.py).
Here is a patch attached.
