OK, I have added all of the changes discussed here (I think) and I now
have a patch series which resolves bugs 750, 1083, 1092, 912, 910 and
593 (I believe) and introduces no test failures (on Windows at least).

I still need to tidy things up, update to crew-tip, and add some
comments before posting, but I will try to do this tomorrow or the
next day.
