[issue992] extdiff: bad handling of added/removed files

If a file is added to a subdirectory which already existed in previous revision,
extdiff should *always* create this subdirectory in the snapshot - potentially
as empty directory. Otherwise the diff program wrongly thinks that the whole
directory has been added.
