[issue944] mq: qpush silently gives up when missing files in patch

New submission from Rusty Russell <rusty at rustcorp.com.au>:

When one file in a patch is missing, you get no warning and future files in
patch are not applied.

You do get:
Errors during apply, please fix and refresh virtio:handle-module-unload.patch

But if there were other rej's, there's no indication that just fixing them up
and refreshing will drop half your patch on the floor.

(If there are no rej messages, you know something is wrong and can use -v and
look for missing files).

This behaviour is a horrible trap; please help!

Thanks!
Rusty.
