[issue1591] Improve "hg help" for extensions

There's not really an obvious way to find out about extensions from the command
line. I'd suggest "hg help extensions" could usefully return a potted version of 

http://www.selenic.com/mercurial/wiki/index.cgi/UsingExtensions

with brief instructions for how to enable them. It's not so easy to find these
things otherwise if you don't know they exist and are already bundled with hg.

Also e.g. "hg help extdiff" could maybe inform me about the extension even if I
haven't enabled it yet.

(On a related note, extdiff seems useful enough to me that it could maybe move
into the core and perhaps become some extra options to diff, which is where I
expected to find this functionality)
