[PATCH 0 of 2] win32text 'none' encoding with tests and docs

This is an update to my previous set of patches for the win32text extension.
They have been made based on discussions with mpm in IRC. Specifically they
take advantage of his new config ordering which just landed (or more
correctly, it no longer needs to take any special action to make the filter
order deterministic), and names the new encoder to 'none:' and makes it
builtin.
The patch includes tests and documentation, including docs for the '!'
behaviour which was adapted from Mad's earlier doc patch.
