[PATCH 00 of 10] Use restructured text for all help

This patch series consists of several patches with the goal of having
restructured text as an intermediate between all the help text and
what is outputted to the screen.

The patch tries to put use rst for more and more of the current help,
and then format each piece. Then the last patch puts these pieces
together, so that one piece of restructured text will be generated and
formatted to display a help text.

There are some changes in output, caused by using optionlist and
fieldlist printed by minirst, instead of just printing directly to the
screen.
