[issue2859] preoutgoing hooks don't pass in the remote URL

New submission from Ehsan Akhgari <ehsan at mozilla.com>:

http://hgbook.red-bean.com/read/handling-repository-events-with-hooks.html#sec:hook:preoutgoing
mentions that the target repo URL is passed to the hook function (or
alternatively via the HG_URL environment variable), but looking at the code,
that's not true.

This makes it impossible to write hooks which only act on one remote repository.
