[issue3014] Extension not working with cmdserver

hg --config 'extensions.fetch=' --encoding=UTF-8 serve --cmdserver pipe
Semms to ignore the config setting. This is very critical this is working in 
order to develop IDE plugins that uses the cmdserver.
