hg graft is failing to commit files that are added in the changeset that is
being grafted.  The file is added to the working copy, and if later grafts
(e.g. from an entire branch) modify that file, it is upgraded to 'M'odified
from 'A'dded, but the file is never committed in any changeset.  

Only merges are committed.  If the grafted changeset includes only 'A'dded
files, I get "graft for revision ##### is empty".

I've tested 2.5.4 and 2.7.0 and the behavior is identical.
