I use Mercurial with the largefiles extension, and intensively branchy
development. Every few days the working copy randomly gets into a state where
several files are listed "R" / added but now missing, and Revert doesn't help,
nor does Update Clean.

The only solution is to update clean to a revision several revisions ago - I
haven't been able to determine a rule, but it tends to be at least 10 revisions
ago; just updating to the previous revision doesn't work.

Updating clean to null also works, but takes a very long time (large repo /
codebase).

This problem occurs using both the command line and using Tortoise, same
version 3.5.1.

I realise this is a bit vague. What can I do, next time this arises, to add
more detail & diagnostics to this report?
