The motivation for a lot of the changes doesn't seem very clear.

Would it be possible for you to break it up into a series of smaller
pieces with explanations?

eg:

patch1 - convert write to take a file descriptor (why?)
patch2 - add http helper functions (why?)
patch3 - make some confusing changes to hgwebdir
..
patch6 - bring in thfcgi.py
patch7 - add fastcgi scripts
