diff missing some files

I'm using extdiff but the following seems to do the same.

hg diff -r1:2

Doesn't show me the files I expect.
If a file didn't change in version1 but did change in version2
I don't get two files to compare with in my visual diff tool.

If it changed in both versions, I get the diff between them.

Running extdiff with a visual diff tool, I expected to be able to diff
between versions.

The temp directories created for each version seem to contain the files that
changed for that version.
I think they should also contain the union of files that changed between
both versions.
