[issue286] mq: "state" shouldn't be in the patch directory

The "state" file shouldn't be in the mq patch directory, since this prevents the
patch directory from being shared by multiple repos.  Similarly, the "series"
file could be separated out.

This would be useful for me, since I'd like to have a single patch repo applied
against multiple other repos in slightly different states.
