cannot commit over an applied mq patch

Hi guys,

A coworker of mine ran into this error while trying to commit a new
patch to a patch queue repository:

> hg commit
abort: cannot commit over an applied mq patch

I have no idea what this means, how it is possible to run into this
situation and how to get out of it. As far as I can tell, the patch
queue repo and the underlying repository look fine.

I only found this code snippet, it looks like some sanity check, but I
have no idea what it means:
