problem with mq extension

Commands like "hg add filename" are not necessary, as I think, if we
already use Mq to manage the changeset.

For example, refer to the Mq wiki page:
http://mercurial.selenic.com/wiki/MqExtension#Command_Examples
We just use "qnew patchname" or "qrefresh", then changes will be commited.
